A new RAND Corporation study concludes that eliminating a key part of health care reform that requires all Americans to have health insurance would sharply lower the number of people gaining coverage, but would not dramatically increase the cost of buying policies through new insurance exchanges.
The study comes as the U.S. Supreme Court prepares to hear arguments in March regarding the constitutionality of the individual mandate, a key provision of 2010's Affordable Care Act.
According to estimates created using a microsimulation model created by the RAND Comprehensive Assessment of Reform Efforts (COMPARE) program, the number of Americans predicted to get coverage in 2016 under the Affordable Care Act would drop from 27 million to 15 million if the individual mandate were eliminated.
Despite that drop, the study estimates that eliminating the individual mandate would increase an individual's cost of buying insurance through the individual exchanges by just 2.4 percent.
Under health care reform, health insurance exchanges will be established to provide policies to individuals who do not have work-based coverage and those who work for small employers. A key issue underlying the relatively small increase in premiums relates to the manner in which premiums in the exchanges are allowed to vary by age.
"Our analysis suggests eliminating the individual mandate would sharply decrease coverage, but it would not send premiums into a 'death spiral' that would make health insurance unaffordable to those who do not qualify for government subsidies," said Christine Eibner, the study's lead author and an economist at RAND, a nonprofit research organization.
Another consequence of repealing the individual mandate would be a sharp increase in the amount of government spending for each person newly enrolled in a health insurance plan. Because most individuals who remain enrolled if the mandate is eliminated are eligible for significant government subsidies, government spending for those newly insured would more than double, rising to $7,468, according to the RAND Health study.
"The individual mandate is critical not only to achieving near-universal health care coverage among Americans, but also to yielding a high value in terms of federal spending to expand coverage," Eibner said. "Without the individual mandate, the government would have to spend more overall to insure a lot fewer people."
One of the most contentious provisions of the 2010 Affordable Care Act is the individual mandate that requires most adult Americans to either obtain health coverage or pay an annual fine. Supporters say the mandate is necessary to encourage young, healthy adults to get coverage, and help spread costs and risks across a larger, healthier group of insured individuals.
If the mandate is eliminated, there is concern that the insurance exchanges established under the Affordable Care Act would suffer from "adverse selection" as only sicker, higher-risk adults would sign up for coverage. Such a trend could lead to higher per-member spending and drive insurance premiums higher.
Several other research groups previously studied what might occur if the individual mandate was eliminated. The RAND study predicts dips in overall insurance coverage and changes in government spending similar to those predicted by the earlier studies.
However, Eibner and co-author Carter C. Price used a more-precise method to estimate the effect that ending the individual mandate might have on insurance premiums. It does not predict price consequences as great as those predicted by others.
Other research teams estimated the overall average increase in premiums that would be expected if the individual mandate is eliminated. The RAND analysis focuses on the annual increase that might be faced by individuals, taking the age of enrollees into account.
Under the Affordable Care Act, individuals cannot be charged higher premiums because of preexisting medical conditions or current health problems. But the law does allow older people to be charged as much as three times more than younger people for health insurance.
The simulations done by RAND and other researchers suggest that most of those who would forgo health coverage if the individual mandate were eliminated would be younger. That would leave the insurance exchanges with proportionately more people purchasing insurance in the older age brackets, where premiums are higher.
Taking these age issues into consideration, RAND researchers estimate that the premiums paid by individuals using the exchanges would be 2.4 percent higher if health reform moves ahead without the individual mandate. This is a more modest impact than others have predicted.
When the individual perspective is dropped, RAND's estimate of the overall average increase is roughly in line with the studies done by other groups. The overall average premium among all people using the exchanges would be 9.3 percent higher -- an increase caused largely because a disproportionate share of people left buying policies after the mandate is repealed would be in older, more-expensive age brackets.

The statue is the first bronze casting of Kim, who during his lifetime shunned proposals to erect a bronze like the massive statue of his father, North Korea founder Kim Il Sung, that towers over downtown Pyongyang. Kim Jong Il, who would have turned 70 on Thursday, died of a heart attack in December.
Kim Jong Il told officials in 1999 that he wasn't ready to accept such adulation while his promise of building an affluent society in the nation of 24 million remained unfulfilled, according to excerpts from a speech published last month. During Kim's reign, the country suffered from a famine that killed hundreds of thousands of North Koreans.
In a posthumous compromise, artisans from the Mansudae Art Studio depicted the Kims riding side by side on horseback for Pyongyang's first public sculpture of the late leader. But artisans told The Associated Press that a towering bronze of Kim Jong Il is in the works and will take its place on Mansu Hill.
Tuesday's widely anticipated unveiling of the 18-foot-tall (5.7 meters) statue took place amid a fervent propaganda campaign to build up the man who led the nation for 17 years as his son and successor, Kim Jong Un, takes over the country's helm.
Kim Jong Il postage stamps, commemorative coins and gold medals have been rushed into production in the weeks before the birthday newly dubbed "Day of the Shining Star." Slogans have been carved on the sides of mountains in honor of his birthday, and a new song composed in his honor.

"Legitimacy comes from his forefathers. Kim Jong Un's father and grandfather may be dead, but he embodies their essence."
Calling himself the "inheritor" of his father's cause, Kim Jong Il was said to have avoided the kind of veneration he ordered for Kim Il Sung, even as he ruled North Korea with an iron fist.
"Some time ago the Party History Institute submitted to me a suggestion that my statue be built marking my 60th birthday," he reportedly told top Workers' Party officials in 1999. "After going over the document, I wrote on it 'Permission Not Granted,' and sent it down."
Kim's era included some of the country's toughest periods, including a famine in the 1990s that killed hundreds of thousands of people and protracted tensions over the nation's drive to build nuclear weapons. Despite a decade of warming ties with South Korea during his rule, relations with Seoul are now at their lowest point in years and the peninsula remains in a technical state of war.
"I cannot have my statue set up on the excuse of my 60th birthday when I have still many important tasks to do, like those of economic construction, improving people's standard of living and reunifying our country," he said, according to excerpts published in the Pyongyang Times last month.
However, he left the matter of a statue to party officials, ordering them to "correctly understand" his desires and dilemma.
Son Kim Jong Un initiated the project two months ago to build the statue of the late leader, said Ro Ik Hwa, a sculptor from the Mansudae Art Studio.
"So far, there has not been any example of making a giant horse-riding statue like this within two months," he told AP. "We sculptors worked day and night to complete it, showing the loyalty of our people."
Kim Jong Un was not at the ceremony attended by the rest of North Korea's core military and political leadership.
The ceremonial head of state, President of the Presidium of the Supreme People's Assembly Kim Yong Nam, praised Kim Jong Il for "applying his unique 'military first' policy to turn our country into an invincible one that can never be defeated and has a nuclear deterrent in the face of the deadly pressing offensive of imperialists."
The unveiling of the statue is the marquee event of birthday commemorations in a nation that remains in a semiofficial state of mourning for the man often referred to as "our fatherly general."

Social Media Week Returns to a City Near You: February 13th - 17th, 2012
Next week marks the launch of the fourth annual Social Media Week, a global event with discussions about media use. I?ll be covering some of the events here on AiP. We?ll return to our regularly scheduled program on Feb. 20th.
The online social world is rapidly developing around us. And there is no longer a choice about participation?not if you hope to be heard. The social media tools that constitute this landscape are diverse and plentiful. It?s easy to feel overwhelmed, however, by the number of services you can find yourself maintaining, easy to feel that it?s impossible to keep up, and easy to give up. But giving up carries a heavy weight. It means reduced connectivity and reduced access to information. And while some may insist that they can get along without these channels, data from the Pew Internet and American Life Project suggests otherwise:

And while event coverage has always been readily available on Twitter, this year global partner Nokia launched a real time infographic tool that can offer targeted information about cities and panels. To help keep participants organized, there?s also an app they can download (though I have mixed feelings about conference apps, since they seem to lose their usefulness post-conference).
As a Social Media Week attendee who has watched this particular event grow successively bigger while retaining its definition and increasing its inclusiveness, it is heartening to see the new discussions that are beginning to happen. Last year, the science community put their toes in the water, and this year, health and wellness will have its own content hub hosted by Saatchi and Saatchi Wellness. PHILADELPHIA (AP) ? Tony Parker scored 37 points, Gary Neal added 18 off the bench and the San Antonio Spurs beat the Philadelphia 76ers 100-90 in a matchup of division leaders Wednesday night.
Tim Duncan had 16 points and 11 rebounds and Tiago Splitter scored 15 for the Southwest Division-leading Spurs.
Lou Williams led Philadelphia with 22 points. Andre Iguodala had 17 and Thaddeus Young scored 16.
The Atlantic Division-leading Sixers fell to 4-2 during a seven-game stretch that coach Doug Collins termed "Death Row" because of the quality of their opponents. After building a strong record early in the season against lesser competition, they beat Orlando, Chicago, Atlanta (on the road) and the Los Angeles Lakers. Their other loss was at home to Miami.
The Spurs are a much different team on the road. But they outplayed the Sixers throughout to improve to 5-8 away from home. They're 13-1 in their own building.
Down 83-71 near the midpoint of the fourth quarter, the Sixers scored seven straight points and had the crowd rocking after Jrue Holiday's 3-pointer. But Neal answered with his fourth 3-pointer of the night and his free throw a minute later after Williams got a technical foul gave the Spurs an 89-80 lead.
Williams hit a 3 to cut the deficit to 95-90 with 1:46 left. But Parker nailed a jumper and Philadelphia couldn't get any closer.
Starting center Spencer Hawes didn't play for the Sixers. Hawes had missed 10 games with an Achilles injury before returning to play the previous two. Elton Brand returned to the lineup after sitting out two games with a sprained thumb, and had 11 points and 13 rebounds.
The Sixers missed Hawes' presence in the middle as the Spurs repeatedly drove to the basket for layups and finished with 54 points in the paint.
The Sixers committed just four turnovers to help them overcome a 55-30 rebounding deficit in a 95-90 win over Kobe Bryant and the Lakers on Monday night. They had four turnovers in the first 11 minutes against the Spurs, and 10 in the game.
Matt Bonner's 3-pointer capped a 17-6 run and gave San Antonio its biggest lead, 71-59, to that point. Williams hit a pair of 3s of his own and the Sixers got within 75-67 after three quarters.
Iguodala kicked off a 10-0 run with a jumper and Young finished it off with a layup that put the Sixers up 38-36 in the second quarter.
Parker scored 10 of San Antonio's next 12 points and the Spurs took a 48-45 lead at halftime. All of San Antonio's 22 points in the second quarter were either on layups or free throws.
Neal, who averages 8.5 points per game, had 12 in the first quarter on three 3-pointers and 4-for-4 shooting ? all in the last half of the period.

Wildlife Refuges Showcase Nature's Courtship Rituals Long Past ...
Washington, DC --(Ammoland.com)- In the wild, some species go all out to woo their mates in spring with noisy and colorful shows. The U.S. Fish and Wildlife Service?s national wildlife refuges are great places to see or hear them.
Here are a few examples of species to look for at refuges across the country:
The male Attwater?s prairie chicken ? a member of the grouse family ? does a jig and makes a ?booming? sound by filling orange air sacks on the sides of its neck. The daytime spectacle is popular with visitors every March and April at the Attwater Prairie Chicken National Wildlife Refuge in Texas, established to protect habitat for the critically endangered bird. See and hear booming here. This year?s annual Attwater?s Prairie Chicken Festival takes place April 14 and 15.
Further north, the American woodcock ? also known as the timberdoodle ? puts on a striking ?sky dance? after dark. Starting at the end of March, the male woodcock leaves its cover for open fields, where it calls to females with a series of sharp ?peent?s. Then it suddenly flies up, twittering, in a widening spiral, floats briefly and dives zigzag back to earth. You can follow it with a flashlight. In his book A Sand County Almanac, famed conservationist Aldo Leopold wrote of the woodcock?s sky dance: ?Since we discovered it, my family and I have been reluctant to miss even a single performance.? Spring woodcock walks are favorites at Missisquoi National Wildlife Refuge in Vermont and Trustom Pond National Wildlife Refuge in Rhode Island. American woodcocks can also be found at Moosehorn National Wildlife Refuge in Maine, Wallkill River National Wildlife Refuge in New Jersey and other refuges. Hear a courting woodcock here.
At Mingo National Wildlife Refuge in Missouri, White River National Wildlife Refuge in Arkansas, and elsewhere, the grey tree frog makes a racket to attract females in spring breeding season. Beginning in early April, it inflates its vocal pouch to balloon-like proportions and emits a melodic trill. And along the Delaware Bay, the annual coming ashore of thousands of horseshoe crabs to spawn is a tourist attraction in May and June. I initially took issue with Dever?s claim that stocks provide no intrinsic return. After all, stocks represent ownership shares of businesses and those businesses earn profits and (ideally) pay dividends.
But while this may be true, Dever correctly points out that the stock market returns realized by investors are driven by two (and in the end, only two) return drivers?the earnings generated by the companies comprising the market and the multiple that investors are willing to pay for those earnings. And except over the very long term, it is primarily investor psychology?the multiple investors are willing to pay?that determines stock returns. On that count, there are not guarantees. Your returns are at the mercy of the fickle and emotional whims of other investors.
As for Myth #2 on long-term, buy-and-hold investing, Dever points out that ?All of the real stock market returns earned over the past 111 years can be attributed to a just an 18 year period?the great bull market that began in August 1982 and ended in August 2000. Without those years, the real, inflation-adjusted returns of stocks, without reinvesting dividends, was negative.? Investors today would no doubt nod their heads in understanding. Excluding the modest returns earned from dividends, investors have seen virtually no returns in over a decade. After adjusting for inflation, the returns are well into negative territory. Dever also correctly points out that for much of the period of time covered in long-term stock market studies, the United States was an ?emerging market,? with the higher rates of growth that this implies.
Furthermore, studies that demonstrate ?market? returns are meaningless because until recent decades, no one bought ?the market.? Index funds didn?t exist, and stock market investing was a hobby primarily of the wealthy, not the middle class. And returns came primarily from dividends. ?Just because something happened in the past does not mean it will reoccur in the future,? writes Dever. ?We must first understand all of the return drivers, and then determine whether those return drivers are still valid.?
